SWCD Liability Wavier for Elk River Cleanup Participants
I hereby acknowledge that during the Elk River Cleanup, neither the Sherburne SWCD nor any other organizer of, or partner in, the Elk River Cleanup is responsible for ensuring in any way the safety of my activities. I understand the inherit risk of some of the activities associated with the Elk River Cleanup, I knowingly and freely assume all such risks, both known and unknown, and assume full responsibility for my participation and personal safety.
I willingly agree to comply with the terms and conditions for participation. If I observe any unusual and significant hazard during my presence or participation, I will remove myself from participation and bring such attention of the nearest official immediately. In the event of an emergency, I authorize Releasees and their officers, agents and/or employees to secure any treatment necessary for my immediate care from any licensed hospital, physician, or medical personnel.
I, for myself and on behalf of my heirs, assigns, personal representatives and next of kin, hereby release, waive, discharge, indemnify, and hold harmless Releasees and their officers, agents and/or employees, other participants, sponsors, advertisers, partners, and, if applicable, owners and lessors of premises used to conduct the event, from any and all claims, demands, losses, and liability arising out of or related to any injury, hospitalization, disability, or death I may suffer. I hereby grant and convey unto the Sherburne SWCD all rights, titles and interest in any and all photographic images, video or audio recordings of me or my likeness.
I hereby acknowledge the following precautions to take during the day of the Elk River Cleanup:
-If taking turns using equipment, be sure to property sanitize hands and equipment after each use
-Wear a mask if in close proximity to other volunteers outside of your household
-Carpool to cleanup locations with only members of the same household
-Maintain social distance of at least 6’ between other volunteers outside of your household
By signing below, I express my understanding and intent to enter into this Release and Waiver of Liability willingly and voluntarily.
